Mychart offers many convenient features, such as scheduling appointments, seeing your test results, printing lab orders, and messaging your provider. However, please be mindful of the length and frequency of Mychart portal messages. Messages regarding medication refills and referrals will most commonly be handled by staff. New medications require an office visit. Please keep in mind that your provider gets many messages per day on top of seeing patients, so please reserve messages for simple, non urgent questions that will get a response within 2-3 days. Please remember that the best health care is provided with a conversation or an in person exam. Most importantly, if you have urgent or emergent symptoms that need addressing the same day, please call the office for an appointment.
Mychart
Prescription Refills
The best way to request a medication refill is through your pharmacy. You can also call the office or send a Mychart message. Please allow 2-3 business days for all prescription refills. Controlled substances will not be refilled after hours or on the weekends and are typically handled by your own primary care provider.
On Call Provider
One of our providers is on call after hours, weekends, and holidays. If you are considering going to the hospital, please call us first so we can help you best triage the situation and coordinate your care. On call is for time sensitive- urgent health care questions and concerns.
Referrals
If your provider recommends a consultation with a specialist or you would like to see a specialist, we will provide a referral. In most cases, we expect to see you first prior to providing a referral. Oftentimes, this is helpful to make sure the referral is appropriate and that we’ve done all of the recommended testing that will be helpful up front.
Forms, Letters, Documents
Please try to bring any forms or documentation that requires a provider’s signature to your appointment. Many times these forms require time & a discussion to fill out. TDI and FMLA forms will not be filled out without a visit. For school/ college forms, please make sure you’ve reviewed the documentation and know what information and/or testing is being requested.
Medical Records
Medical records are released when we are provided with a signed release form from the patient or legal guardian. We are responsible for the protection of these records and must have proper documentation for release.
There is a $20.00 medical record request fee. Payments accepted are cash or check only. Make out a check to: SHARECARE. Please see “Medical Records Release” under our Forms section. Please return the form and cash or check to our office for processing.
Billing & Insurance
We accept most major insurance plans, with the exception of some out of state plans.
