Please try to cancel routine visits at least 24 hours in advance. If you are sick at the time of your annual exam, we ask that you call in advance to let us know. There is a $50 fee for missed physical appointments and $25 fee for missed sick visits.
Please be advised that if you have not been seen in over 3 years, you are automatically discharged from our system and considered a new patient (subject to availability).
Please bring your insurance card and license to every visit.
Please make sure to update us if your insurance information has changed.
Please make sure to update your insurance company with the correct PCP.
Copayments are collected at the time of your visit.
